To obtain a duplicate of a Czech Birth Certificate please send the following:

1.

filled out the application form, your signature on the application form must be verified by notary public

2.

copy of your passport or driver's license

3.

in case you are relative, you need to provide original documents proving it, in case you are not relative, enclose explanation letter why do you need the birth    certificate

For the information about the fee, please click HERE. You can pay in cash or money order. Unfortunately, the Consulate cannot accept the personal checks.

If you wish to apply for the Apostille, you need to include additional fee which is under number 150e

5.

filled out the  Consent to the processing of personal data (PDF, 554 KB)

If you would like us to send your Czech document to your address, please send us a pre-paid USPS Priority Express Mail envelope  - EP13F (only overnight express mail with a tracking number).

For future tracking, we suggest you to make a record of the tracking number.
