To proceed the duplicate of the birth certificate you have to present:

1. completed application form (your signature on the application form must be verified by a notary public)

2. copy of your passport or drivers licence

3. in case you are not a relative, enclose an explanation letter why do you need the duplicate of the birth certificate

4. fee

• the fee for issuance of the duplicate of the birth certificate
• the fee for issuance of an apostille please add extra

The General Consulate of the Czech Republic is accepting only a cash or  money order as a form of payment of the fee. The General Consulate of the Czech Republic is not accepting personal checks or credit cards.
