Visa for executive manager purposes

– participation in a legal entity established under Act No. 513/1991 Coll.
Particulars of the application for issue of the visa
- a travel document;
- 2 photograph;
- a completed form “Application for long-stay Visa”;
- funds to cover the cost of stay in the territory;
- a document confirming the purpose of stay in the territory of the Czech Republic. Such documents include, in particular:
- a statutory body or a member of the statutory body of a company who does not ensure for the company the performance of tasks arising from the object of business activities of the company as the legal entity:
o shall present a copy of the excerpt from the Commercial Register indicating that he/she fulfils the relevant conditions; if he/she is unable to present a copy of the excerpt from the Commercial Register because he/she has not yet been registered there since he does not possess the relevant residence permit, he/she will present a certified copy of the memorandum of association or of the notarial deed on founding or change of the company, or in case of a limited liability or a joint stock company also a certified copy of its articles of association (provided that they are to be issued under the memorandum of association) indicating that the alien meets such conditions. The fact that the alien does not ensure for the company the performance of tasks arising from the object of business activities of the legal entity results from the application; or
- a member of a statutory body of a cooperative who does not ensure for the cooperative the performance of tasks arising from the object of business activities of the legal entity:
o shall present a copy of the excerpt from the Commercial Register indicating that he/she fulfils the relevant conditions; if he/she is unable to present a copy of the excerpt from the Commercial Register because he/she has not yet been registered there since he does not possess the relevant residence permit, he/she will present a certified copy of the record of founding or change of the cooperative or a certified copy of the notarial deed on appointing a member of the cooperative as a statutory body by the meeting of members, indicating that the alien fulfils such condition; or
- a liquidator, bankruptcy trustee, composition trustee, receiver or an alien who is authorized to bind or to represent the company before the court or to participate in the management of or control of the company as a body established by the law or as a member of such body, and who does not ensure for the company or for the cooperative the performance of tasks arising from the object of business activities of the legal entity:
o shall present a certified copy of an excerpt from the Commercial Register or another document indicating that he/she meets such conditions;
- a document confirming the availability of accommodation for the period of stay in the territory of the Czech Republic.
1. The document confirming the availability of accommodation means a document by which you prove the availability of accommodation and which indicates that the accommodation will be provided for the required period of stay from- to or for an unlimited period, i.e.:
1.1 an original or a certified copy of an accommodation, lease or sublease agreement;
1.2 a certificate issued by an individual or a legal entity providing accommodation to you, including a certificate issued to the alien by the party which accommodates him (pursuant to Section 100(e) of Act No. 326/1999 Coll.); such document may be also produced by an alien below 15 years of age who is not stated in the agreement referred to in paragraph 1.1.;
1.3 an authenticated Invitation form in case of a temporary stay;
1.4 an original or a certified copy of an excerpt from the real estate cadastre, if you own real property; or
1.5 if you commute to work from a neighbouring state, a written confirmation of accommodation in the territory of the neighbouring state from which you commute daily to work in the Czech Republic (need not be certified);
- an excerpt from the Penal Register (or another similar document) issued by the state whose national you are, as well as by states where you stayed for more than 6 consecutive months in the last 3 years. This shall not apply to an alien younger than 15 years of age; a foreign document must be appropriately certified unless stipulated otherwise by an international treaty that is binding for the Czech Republic, and accompanied by an official translation into Czech;
- a medical report certifying that you do not suffer from a serious disease (if requested by the DM);
- travel medical insurance for an amount of at least 30,000 EUR, covering the territory of the Czech Republic (such insurance shall be presented before affixing the visa in the passport);
a proof of payment of the fee for submission of the visa application
