Long-term residence permit for the purpose of family reunification in the territory

Family reunion
You may file an application for a long-term residence permit for the purpose of family reunification in the territory (the “family reunification”) if you are:
- the spouse of an alien holding a residence permit;
- a minor or an adult dependent child of an alien holding a residence permit;
- a minor or an adult dependent child of the spouse of an alien holding a residence permit;
- a minor alien who has been entrusted by a relevant authority's decision into substitute family care of an alien holding a residence permit or of his/her spouse, or who has been adopted by an alien holding a residence permit or his/her spouse or whose guardian is an alien holding a residence permit or his/her spouse, if the care for the minor alien is to take place in the territory;
- a single alien older than 65 years of age or, regardless of your age, an alien who cannot take care of himself due to medical reasons in case of family reunification with a parent or a child holding a residence permit in the territory.
An application for long-term residence for the purpose of family reunification is filed at a diplomatic mission. Aliens who stay in the territory on the basis of a visa for stay over 90 days or of a long-term residence permit issued for another purpose may file an application for long-term residence permit for the purpose of family reunification with the police in the territory of the Czech Republic.
Particulars of the application:
- a completed application form;
- a document of availability of accommodation during the stay in the territory;
- a travel document;
- two photographs;
- a proof of the family relation;
- the consent of a parent or other legal representative or guardian with the child's stay in the territory (if such reunification does not represent reunification of the family with such parent, legal representative or guardian);
- a document confirming that the sum of the family income after reunification will not be less than the sum of:
- the subsistence minimum amounts of all family members; and
- the maximum amount of standardized housing costs determined by a special law for housing contribution purposes or the amount you prove by credible evidence as the amount of actual justified cost spent on family housing;
- an excerpt from the Penal Register to assess your criminal record, issued by the state whose national you are, as well as by states where you stayed for more than 6 consecutive months in the last 3 years. This shall not apply to an alien younger than 15 years of age; a foreign document must be appropriately certified unless stipulated otherwise by an international treaty that is binding for the Czech Republic, and accompanied by an official translation into Czech;
- a medical report certifying that you do not suffer from a serious disease (if requested);
- travel medical insurance for an amount of at least 60,000 EUR, covering the insurance in the Czech Republic (such insurance shall be presented before affixing the visa in the passport; however, we recommend presenting the insurance at the time of submission of the visa application); the medical insurance contract and the information about insurance conditions should be translated by a certified translator;
- a proof of payment of the fee for submission of the application for a long-term residence permit
