Czech project for long-term visa (over 90 days) for purposes of the spa-rehabilitation treatment (accelerated procedures)

- The project concerns only patients of certain providers of the spa-rehabilitation treatment (Lázně Darkov, Léčebné lázně Jáchymov, Thermal-F, Lázně Teplice v Čechách) under specific conditions, citizens of the United Arab Emirates, Saudi Arabia, State of Kuwait, State of Qatar, the Kingdom of Bahrain and the Sultanate of Oman, whose health condition requires a long-term treatment.
- Applies only on patients, who are suffering from the illnesses specified under the Decree No. 267/2012 Coll., on the indicative list for the spa-rehabilitation treatment for the adults, children and the youth (as amended by later amendments), and at the same time not suffering from the illnesses specified under the Decree No. 274/2004 Coll., on the list of illnesses, which could be a danger for public health (as amended by later amendments).
- A patient can be accompanied by 1 accompanying person (only if there is a strong specified reason given in written by the patient himself maximum 2 accompanying persons could be allowed) such as parents/legal guardians in case of minors or personal assistant in case of adult of minor patient, whose health condition requires permanent assistance.
- The acceptance of the patient for a long-term treatment must be confirmed to the Embassy directly by the spa from the Czech Republic.
- The patient has to submit a c o m p l e t e application for issuing of the long-term visa (the particulars of the application for the issue of the visa can be found in the attachment below).
- For the accompanying person(s), the relevant forms have to be filled in and included (the forms will be provided by the Embassy).
- The applicant has to provide a pre-paid envelope of the express post services (e. g. DHL or ARAMEX), by which the application will be sent to the authorities in the Czech Republic for the express decision-taking process (the decision should be taken up to 1 month). In case he/she fails to deliver it within 3 days, a standard decision-taking process (up to 3 months) will be taken.
- A proof of payment of the fee for submission of the visa application.
IMPORTANT NOTICE: The long-term visa is in principle issued as Czech national visa. Only Schengen (short-term visa) provides you with an access to the Schengen area. If the visa has been issued only for the Czech Republic, than such a visa provides you with an access only to the Czech Republic.
