Visa for Employment and Other Long Term Visas exceeding 90 days

REQUIREMENTS:

In order to process this type of visa application, the following documents are required:

1. Application form -filled out and signed. Visa Application Form - download - available under Visa Information

2. Two photos.

3. Original passport and the copy of the first page.
The original passport may be submitted after your visa has been approved; in this case, please submit a copy of the original passport (page with your name and photograph).

4. A notarized copy of a valid credit card (both sides).
(This document is a proof of financial support for the duration of your stay in the Czech Republic, you can white out part of the card number ).

5. Document confirming the reason for your stayin the Czech Republic - all documents must be written or translated to Czech!

(Find what applies to your case and submit the documents accordingly)

a) employment - a notarized copy of a work permit issued by the Department of Labor;
b) family- a notarized copy of a marriage certificate in husband/wife cases; a notarized copy of a birth certificate in parent/child cases. Apostille is required for both of these documents.
c) business - a notarized copy of your registration in the company records;

NOTE: for any other purpose of stay such as medical, cultural, sport, family, official/state visit, internships or long term tourism please contact the Consulate for information.

6. A document proving your accommodation arrangements:

A statement with the notarized signature of the person providing your accommodation, which contains their agreement to your lodging for your stay in the Czech Republic. The following are acceptable documents:

a)in cases where the property is owned by a person: a declaration stating that he/she will be providing you with lodging;
b) in cases where the property is owned by a legal entity: a declaration stating that it will be providing you with lodging;
c) in cases of renting an apartment or other property: the original lease or its notarized copy;
d) in cases of subletting: the original contract for the sublet and a written agreement of the owner of that property (that is the lessor) or their notarized copies;
e) in cases where the accommodation is being used for study purposes, it is possible to submit documents issued by government bodies or legal entities confirming your accommodation during your stay for study.

7. Affidavit (waived for applicants younger than 15 years of age)

a)covering the U.S.A: an Affidavit , the text of which is enclosed both in English and Czech languages. It is necessary to fill out and sign the affidavit and then have your signature notarized by the Consulate or a notary public;

b)covering the Czech Republic: submit copy of your birth certificate and marriage certificate

8. For applicants of Czech Origin only: applicants born in the territory of the Czech Republic (or in cases where the mother or the father of the applicant was born in the Czech Republic) are required to present a document proving the date when he/she became a U.S. citizen: (U.S. Naturalization Certificate).

9. ) Proof of Travel/Health Insurance - for the whole period of stay covering possible expenses and hospitalization costs for medical reasons (accident or unexpected illness) while in the Czech Republic, including repatriation to the state which issued your travel document or to the state in which you have a legal residence. Minimum coverage must be at least 45 000 USD. Please provide a photocopy of such document.

Current Administration Fees:

The fee for a Long Term Vvisa (visa exceeding 90 days) - $131.00

Please include a prepaid self-address envelope (preferebly with a tracking number)

All fees are paid at the time of submitting the application.

WHEN SUBMITTING THE APPLICATION FORM AND SUPPORTING DOCUMENTS ALSO PLEASE INCLUDE COPIES OF ALL THE SUBMITTED DOCUMENTS AND COPY OF THE APPLICATION FORM AS WELL

Important note for all applicants!

The Czech Immigration Office may always request additional documents and/or information before issuing the visa, therefore always include your home or office telephone number and e-mail address!!!
