General information on birth, marriage and death certificates

All vital events (i.e. birth, marriage, death) of Czech citizens are registered by the office of "Zvláštní matrika" in the city of Brno, Czech Republic. Applications for birth certificates, marriage certificates and death certificates can be submitted to this office directly or through our embassy.

General information

To register a vital event taking place in Canada one must submit the appropriate Canadian vital statistics document, (1) apostilled and (2) translated into Czech language (3) the translation to the Czech language will be verified while processing your application.

How to submit the application?

In person - you need to make an appointment for a personal submission by sending the request to the email address Ottawa.Consulate@mzv.gov.cz

Applicant's signature on each application form must be verified by the honorary consul of the Czech Republic in Canada, when mailing the application.

We would also appreciate receiving a pre-paid trackable envelope with your return address by which we will send your document back to your address. We recommend using Xpresspost envelopes. If you do not submit it, the issued Czech document will be sent back to you by a regular mail.

The consular staff will help you to clarify any questions, but they cannot fill out the forms for applicants.

Honorary consulates are not entitled to deal with registry and citizenship issues - in exceptional cases, due to considerable geographical distances, honorary consul can verify the signatures of the applicant on applications forms. Honorary consul  is not responsible for the correctness of submitted applications and it is therefore necessary for applicants to contact the Embassy of the Czech Republic in Ottawa with all questions they may have regarding their application.

All copies of the documents attached to the application must also be verified, unless you are also sending their originals. Copies of documents can be verified either by the honorary consulates of the Czech Republic in Canada (then no further verification is required) or by a Canadian notary, in which case they have to be appostilled. Copies of documents verified only by a Canadian notary public will not be accepted. Documents in English have to be accompanied by the translation into the Czech language, which will be verified for a fee by our office, while processing your application.

When sending an application by post:

  • one of the forms of payment of the consular fee for the application and verifications of documents (if applicable) must be  enclosed
  • a pre-paid Xpresspost envelope should be enclosed. If the prepaid envelope is not sent together with the application, the applicant must pick up the issued document in person at the consular section (after arranging an appointment at consular_ottawa@embassy.mzv.cz), or the document will be sent back by the regular mail.
