Birth Certificate

How to submit the application?

In person - you need to make an appointment for a personal submission by sending the request to the email address Ottawa.Consulate@mzv.gov.cz

When mailing the applicatiom, your signature on the application form has to be verified by a Czech honorary consulate in Canada.

The application consists of:

  1. Proof of child's Czech citizenship  (either a certificate of Czech citizenship not older than 1 year, or valid passport or valid national ID card), or  a completed application and questionnaire for establishing Czech citizenship and issuing a certificate of Czech citizenship,. In case of registration of the birth of a child who is under 1 year old on the date of the submissiion of his application for a birth certiticate, instead of a certificate of Czech citizenship or a completed application and questionnaire for establishing Czech citizenship and issuing a certificate of Czech citizenship, it can be submitted for the child born
    • - for the duration of the marriage, a document proving the Czech citizenship of the child's mother or father on the day of the child's birth, or
    • - outside of marriage, a document proving the Czech citizenship of the child's mother on the day of the child's birth;
    • - this procedure cannot therefore be applied in cases where the only citizen of the Czech Republic is the father of a child born out of wedlock.
  2. Application form Zápis o narození (DOCX, 40 KB);
  3. Canadian birth certificate in the long-form version, apostilled and translated into Czech;
  4. Czech marriage certificate of the applicant's parents. If the parents were married in Canada and their marriage has not yet been registered in the Czech Republic, their Canadian marriage certificate must be submitted apostilled and translated;
  5. Personal data (DOC, 24 KB) form;
  6. Consular fee for the application. Please take note, that other consular fees might be connected with the processing of this application, namely for documents verifications (e.g. verification of copies or translations).

We would also appreciate receiving a pre-paid trackable envelope with your return address. We recommend using Xpresspost envelopes.

Special requirements

- If the mother of the applicant is divorced, she must submit her marriage certificate and proof of the divorce with an official timestamp confirming when the divorce took effect or a legally effective decision of the "Nejvyšší soud ČR" approbating the foreign divorce;

- If the mother of the applicant is a widow, a birth and death certificate of the husband must be submitted;

- If the mother of the applicant is single or the parents of the applicant are not married, a form Souhlasné prohlášení o otcovství (PDF, 327 KB) and birth certificate of the mother must be submitted;

- If the applicant is a female child, she can register the surname in a neutral form based on the application form Zápis příjmení v mužském tvaru (PDF, 320 KB). It is also possible to apply for the registration of the surname in neutral form additionally, by filling out this form (PDF, 320 KB)

- If the applicant is a female adult, she also can by this form (PDF, 310 KB) register her surname in neutral form. The registration of her surname in neutral form can be applied for additionally as well, by using this form (PDF, 311 KB).
