Application for a duplicate of a Czech Birth, Marriage or Death Certificate

 

To obtain a duplicate of Certificate please send us the following:

1. completed aplication form (your signature on the application form must be verified by a Justice of Peace)

2. verified (by Justice of Peace) copy of your passport

3. money for fee

• the fee (code 153a for issuance of the duplicate  and code 150e if apostille is also required)

The Consulate of the Czech Republic is accepting only a cash,  money order or bank cheque as a form of payment of the fee. The Consulate of the Czech Republic is not accepting personal checks or credit cards.
