LIST OF DOCUMENTS

needed for accreditation in the Embassy of the Czech Republic in Tbilisi

1. Detailed information letter about the Company

- its history, activities, experience;

- its Charter (not obligatory but welcomed);

- its previous cooperation with Czech/international partners (resp. other Embassies in Tbilisi);

- expected number of Georgien/Armenien tourists wishing to visit the Czech Republic through the Company in the nearest future, etc. 

2. Valid Licence for tourism services

- original or copy certified by a notary public + translated into Russian/English (authenticated translation)

3. State/Trade Registration Certificate

- original or copy certified by a notary public + translated into Russian/English (authenticated translation)

4. Extract/Confirmation from the Ministry of Finance - Tax Department (or any other appropriate institution) about paying the taxes

- original or copy certified by a notary public + translated into Russian/English (authenticated translation)

5. Contract with bank

- original or copy certified by a notary public + translated into Russian/English (authenticated translation)

6. Contract with airlines company

- original or copy certified by a notary public + translated into Russian/English (authenticated translation)

7. Contract with insurance company

- original or copy (authenticated translation)

8. Valid contract with Czech partner

- original or copy certified by a notary public

9. Licence and Trade Register Extract of the Czech partners

- original or copy certified by a notary public

10. List of couriers (max. 3) allowed to bring the visa application to the Embassy (including their photos, contacts and passport/ID card copies)
