Duplicate of Vital Statistics Certificate

If you do not possess the vital statistics certificate, it is possible to apply for a duplicate. Duplicates can be issued only if the birth, death or marriage was registered in the Czech Republic.
 

Eligibility:

  • Person listed on the document
  • Relative – this must be proven by submitting a vital statistics certificate, for example birth certificate of a child who is applying for a parent‘ s certificate, a marriage certificate in case of spouse etc.
  • Other person who demonstrates a legal claim

 

Steps to follow:

  1. The application is submitted in person during an appointment that has been arranged beforehand by e-mailing: toronto.appointment(at)mzv.gov.cz. If applications are mailed, the applicant’s signatures on the forms MUST be verified by the Honorary Consulate in your area.
  2. Application forms must be filled out correctly, legibly, in Czech language and nothing is to be omitted. Dates are to be written in the format Day/Month/Year. The geographical names must be in Czech language.
  3. If required, it is possible to request Apostille if the document is to be used in a country that recognizes/requests Apostille OR request a multiple EU language glossary if the document is to be used anywhere in the EU. There is an extra cost associated with those.
  4. You will need to submit a valid picture ID and the form Osobni udaje /Contact information form
  5. The certificate can be picked up in person or if the certificate is to be mailed, A prepaid, self-addressed envelope XpressPost is required at the time of the application.
