If you are born abroad and were just issued your first Czech citizenship certificate, registering your birth and possibly marriage is the essential step towards obtaining a Czech passport and other Czech identity documents. Czech citizens are obliged to present Czech issued registry certificates in the Czech Republic; therefore, it is advisable to submit an appropriate application as soon as possible and not delay it. Life events of Czech citizens that took place abroad are registered by Zvláštní matrika Brno. Applications can be submitted at our office.

Please note: if you are looking to obtain a replacement certificate - i.e., the event had already been registered in the CR, consult the information on issuing duplicates here.

The key documents required to register a life event are the official foreign vital registry document, proof of Czech citizenship of the person whose life event is to be registered and other additional documents required to verify the accuracy of the information being registered. You may notice that some of the required documents to register a life event overlap with the documents necessary to apply for a Czech citizenship certificate. 

General steps to follow:

·       The application is submitted in person during an appointment that has been arranged beforehand by e-mailing: toronto.appointment(at)mzv.gov.cz

Under the age of 18 - applications are submitted by a parent or a legal guardian  
Over the age of 18 - applications are submitted by applicants themselves

·       If applications are mailed, the applicant’s signatures on the forms MUST be verified by the Honorary Consulate in your area. PLEASE NOTE, if you are submitting a photocopy of a Czech passport or obcansky prukaz, the holder must date and sign the copy and include a written acknowledgment that the ID has been photocopied for the purpose of the application: "Souhlasím s pořízením kopie mého dokladu“

·       Application forms must be filled out correctly, legibly, in Czech language and nothing is to be omitted. 

Leaving items blank delays the processing and may result in the application being returned. If there is an item that is not applicable to your situation, it must be indicated on the form. Dates are to be written in the format Day/Month/Year.
Names must be written out completely, no abbreviations.
Places of birth must be in the format Town/Country

A Consular officer will review the form and may be able to answer your questions but is unable to fill the forms out for you.
