As of 1 September 2021, only COVID-19 vaccination, immunity or contraindication certificate holders may visit the Czech Embassy in Baku

In accordance with the measures implemented at the territory of the Republic of Azerbaijan in order to prevent a further spread of COVID-19, only holders of certificates including a QR code and proving vaccination or immunity against this disease or exemption due to contraindications to COVID-19 vaccines may visit the Embassy of the Czech Republic in Baku, including its Consular and Visa Section, as of 1 September 2021. Minors under 18 years of age are exempt from this rule.

In the Republic of Azerbaijan, a vaccination or immunity certificate may be obtained via my.Gov.az. To receive a vaccination exemption (contraindication) certificate, it is necessary to contact a polyclinic in the area of residence at the territory of the Republic of Azerbaijan to submit necessary information to a special commission set up by the Ministry of Healthcare which, in turn, decides whether a certificate will be issued or not.
