Czech Criminal History Record
The Czech Criminal History Record is issued by the criminal registry authority "Rejstrik trestu" in the Czech Republic. For those who currently reside in the USA it is possible to apply for it at the Embassy of the Czech Republic in Washington, DC, or at the Consulates General of the Czech Republic (New York City, Chicago, Los Angeles). Please check if your permanent residency belongs to a jurisdiction of the Consulate General of the Czech Republic in New York.
To obtain the Criminal History Record you need to:
1) complete the following fields of the Application form:
- name and surname at birth
- present surname
- date of birth
- place of Birth
- personal number - for those born in the Czech Republic
- name and surname of father
- name and surname of mother
- mother's maiden name
- Your signature verified by Notary Public
1a) If you are a citizen of the Czech Republic you need to sign the Approval for accessing your personal data
2) Send along a notarized copy of your Birth Certificate and your ID such as passport or driver's license.
3) The fee for issuing of an Excerpt from Czech Criminal History Record (item 162b) is payable in Cash, Money Order or Cashier's Check to the "Consulate General of the Czech Republic in New York". The Consulate does not accept personal checks.
4) Please, enclose a pre-paid, self-addressed Certified or Express Mail envelope with your home address in USA (for future tracking, we suggest making of written record of the tracking number of the envelope). The Consulate will not accept any responsibility for lost or damaged articles. We recommend using:
For overnight service
USPS Priority Mail Express Flat Rate Envelope - EP13F together with USPS Label 11B and sufficient stamp.
For priority mail delivery
USPS Priority Flat Rate Envelope – EP14F together with USPS Tracking # - Label 400 and sufficient stamp.
Alternatively, you can print shipping label by using Click-N-Ship® service.

In most cases, the document is issued on a same day. In complicated cases, we may forward the application to the Czech Republic, which may take up to 4-8 weeks. After the arrival of the document to the Czech Consulate, we will mail it to you immediately.
