Extract from the Czech Criminal Register

It is possible to obtain an Extract from the Czech Criminal Register via Czech POINT at the consular section of the Embassy of the Czech Republic in The Hague.

You can submit the application in person during office hours of the consular section of the Embassy (Monday 12 – 3 p.m., Tuesday and Wednesday 9 – 11:30 a.m.). Fees can be paid in cash in EUR or by payment card, the current fee can be found on our website in the chapter Consular Fees.

You have to prove your identity with a valid passport or I.D. document (but not a driving license).

Foreigners have to prove also his/her surname at birth by another document (e.g. birth or marriage certificate), if it is not listed in the passport or I.D. document. In this case contact us before submitting the application by tel.: (00-31) 070 313 00 18 or e-mail: consulate_hague@mzv.cz.

The record for foreigners is not issued on the spot, but approx. within 25 days.

Note: An extract from the Czech Criminal Register is issued in the Czech language, on request also with the multilingual standard form according to Regulation (EU) 2016/1191 of the European Parliament and of the Council of 6 July 2016. The extract issued via Czech POINT cannot be certified with an apostille.  In case you need the extract with an apostille, please apply for it directly in person at the Criminal Register Authority in Prague: Rejstřík trestů ČR, Soudní 1, Praha 4, phone no.:+420- 244 006 111, https://portal.justice.cz/Justice2/Soud/soud.aspx?o=203&j=213&k=2027&d=9354. They issue a record with a blue square stamp in the left down corner, which can be further verified by an apostille by the Ministry of Foreign Affairs of the Czech Republic.
