Required Documents in Visa Application for Erasmus Students

Consulate General of the Czech Republic in Istanbul announces that in accordance with the new legislation No.: 233/2019, Long Term Visa and Long Term Stay Applications will be accepted in accordance with the applicant´s permanent residence address (in case of students not the city where the university of the student is located in Turkey). Therefore, the applicants should attach the scan copy of their residence address registry document, which can be generated on e-devlet system to their email for appointment request. * PASSPORT: It must be valid for at least 90 days (3 months) from the departure date from the Czech Republic, it must have at least 2 empty pages, it must be issued within the last 10 years. Those who have a family name change after marriage should apply by taking their new passport according to their new surname within 3 months from the date of marriage. Residence permit of foreign national Erasmus students must be valid for at least 1 year from the day of their return from the Czech Republic.

 

* VISA APPLICATION FORM (LONG TERM): It is the form which is required to be filled by the students who will apply for one semester and two semesters to Erasmus Program. The form must be completed in accordance with the example form and fully prepared to the appointment. Click here for the sample form.

 

* PHOTOGRAPS: Two photographs are required for long-term visa applications. Photos must be taken 35 x 45mm in size, head size not larger than 35 mm, and taken within the last 6 months, from upright and white background. Name, surname and date of birth should be written on the back of the photographs.

 

* LETTER of ACCEPTANCE: (Jednotné potvrzení o studiu) It must be received from the university in the Czech Republic as original, wet-signed and written in Czech. It must include the applicant's name and surname. Scanned copies, photocopy or fax are not accepted. The documents which are not received in Czech language must be provided with its Czech-translation. 

 

* ACCOMODATION LETTER: (Potvrzení o ubytování, Smlouva o ubytování). ) It must be received from the university in the Czech Republic as original, wet-signed and written in Czech. The accommodation documents received in the form of contract must have original signature and be signed by the applicant in front of the consulate.

 

* ERASMUS PARTICIPATION LETTER: This is the document approving the participation of the student to the Erasmus program and the amount of grant received by the student. This document must include name and surname, must be original, wet signed and be submitted with Czech translation if not prepared in Czech. The students who are granted or the students whose grant is not approved yet during the application process must submit a bank account document showing the last 3 months of movements besides Erasmus Particiation Letter. This document must have been opened for their respective name and include wet signature and original stamp. The account in question must include 300 euro per month or its equivalent in Turkish currency - Detail information. The document must be translated into Czech. Students are also supposed to submit the photocopy of their bank card belonging to the same bank account. This bank card must have the respective name of the student on the front side. 

 

* CRIMINAL RECORD LIST: All Erasmus students must submit the criminal record list including date of birth and place of birth and taken from court house or district governorship with apostille stamp. Both the document and the apostille stamp must be translated into Czech and submited together with original documents. Students who are not Turkish citizen and the applicants who stayed in abroad more than 6 months within last 3 years must also receive criminal record from those countries with apostille stamp and its Czech translation in addition to the criminal record received from Turkey. 

 

* HEALTH INSURANCE: Health Insurance is not required during the visa application but after the visa has been approved. Applicants who have been covered by the SSI (SGK) from their families are required to submit the TR / CZ 111 Insurance Certificate to be obtained from the Provincial Directorates of SSI. Those who are out of this context are required to submit a Comprehensive Insurance Policy, whose amount is exactly 60.000 Euro including the whole accomodation process, that is valid in the borders of Czech Republic. The list of valid Czech Insurance Companies are sent as e-mail to be submitted by the applicants. The Insurance policies done by Turkish companies in Turkey is not acceptable. More detail will be given to the appliacant via e mail after the visa process.

* Other Requirements: 

o Photocopies of passport with pages invlove old visas and enty-exit stamps, if any, on an A4 paper.

o Do not have the notary approval of the documents translated into Czech. Approvals of the translations will be made by our Consulate.

o All documents listed above are requested to be submitted with one photocopy.
