Application for a Schengen (short-stay) visa is submitted on an official form “Application for Schengen Visa.” The application form can be printed by clicking on the respective language mutation. The application form can be printed in black and white. Diplomatic missions provide application forms as well. Diplomatic missions also accept application forms used in another Schengen state, provided that they comply with the Schengen standards.

Application form for Employee Card:

An employee card can be applied for by submitting a special application (zamka (PDF, 482 KB) where you must also state the number of the job vacancy according to the central register of job vacancies that can be filled by holders of employee cards, in addition to providing the relevant information.
