1. BEFORE YOU TRAVEL, FILL OUT A FORM FOR HYGIENIC STATION

Australia, Belgium, Bulgaria, Finland, Greece, Holy See, Iceland, Japan, Luxembourg, New Zealand, Norway, Singapore, South Korea, Thailand
OR IF YOU ARE
● employed in international transportation
● EU citizen or third country citizen with long-term or permanent residence in the EU transiting through the Czech Republic for less than 12 hours
● holder of diplomatic passport or international organization staff if the stay will not exceed 14 days
● pupil, student, pendler or other cross-border worker who crosses the border regularly at least once a week
YOU NEED TO FILL OUT THE FORM IF YOU ARE ARRIVING FROM ANY OTHER COUNTRY EXCEPT FROM ONE OF THOSE LISTED ABOVE AND IF YOU HAVE STAYED IN THIS OTHER COUNTRY FOR MORE THAN 12 HOURS IN THE PAST 14 DAYS

Children younger than 18 accompanied by a family member do not need to fill out their own form – the accompanying adult will register them by filling out the field about minor co-travelers.
Explanation: The countries listed are on the "green list" meaning they are considered to be low-risk in terms of COVID-19 spread.

2. PREPARE A CONFIRMATION FOR PASSPORT CONTROL
Once you fill out and submit the form, you will receive a confirmation e-mail. You may be asked to show this confirmation when passing through the passport control.
You may either print out the confirmation or display it on your mobile. The confirmation e-mail will containing a QR code that will display an on-line version of the confirmation.
Do keep the confirmation on you while in the Czech Republic.
3. GET TESTED IMMEDIATELY AFTER ARRIVAL
YOU MUST GET TESTED FOR COVID-19 IMMEDIATELY AFTER ARRIVAL, AT MOST WITHIN FIVE DAYS OF ARRIVAL - unless arriving from one of the countries stated above, i.e. from the "green countries."
We suggest to make every attempt to schedule your test appointment before you travel.
Please plan ahead and avoid waiting lines!
HOW DO I FIND A TEST SITE?Refer to the LIST OF TEST SITES regularly updated by the Ministry of Health. The list is available in Czech only. We suggest to ask your inviting party to help you find the nearest test site. You may also seek assistance with the coronavirus help line 1221 (Mo-Fri 8 am - 7 pm and Sat-Sun 9 am 4:30 pm) or with the help line of the Ministry of Health: + 420 224 972 222 (Mo-Fri 9 am - 5 pm), e-mail verejnost@mzcr.cz. 4. INFORM THE REGIONAL HYGIENIC STATION ABOUT YOUR TEST RESULT
You are obligated to contact the Regional Hygienic Station according to your place of stay within seven days after arrival and report your test result.
