Medical Certificates for Persons, who have been vaccinated in Slovenia or have suffered from COVID-19 there, for the Purpose of entering the Czech Republic

Persons, who have been vaccinated in the Republic of Slovenia or have suffered from COVID-19 there, are for the purpose of entering the Czech Republic without quarantine or to utilize services in the Czech Republic, required to present themselves with the Medical Certificate attached below or with the International Certificate of Vaccination or with one of the Certificates recognized by the Ministry of Health of the Czech Republic.
