A) Duplicate of a Czech Birth Certificate

 

To proceed the duplicate of the birth certificate you have to present:

1. Completed APPLICATION FOR DUPLICATE OF BIRTH CERTIFICATE (PDF, 109 KB) (your signature on the application form must be officially verified)
2. Copy of your passport or driving license
3. In case you are not a relative, enclose an explanation letter why do you need the duplicate of the birth certificate
4. Fee

• the fee for issuance of the duplicate of the birth certificate
• the fee for issuance of a verification please add extra

The Embassy of the Czech Republic is accepting only cash as a form of payment of the fee. The Embassy of the Czech Republic is not accepting personal checks or credit cards.

B) Application for a duplicate of a Czech Marriage Certificate

 

To obtain a duplicate of a Czech Marriage Certificate please send us the following:

1. Completed APPLICATION FOR DUPLICATE OF MARRIAGE CERTIFICATE (PDF, 209 KB) (your signature on the application form must be officially verified)
2. Copy of your passport or driving licensee
3. In case you are not a relative, enclose an explanation letter why do you need the duplicate of the marriage certificate
4. Fee

• the fee for issuance of the duplicate of the marriage certificate
• the fee for issuance of a verification please add extra

C) Application for a duplicate of a Czech Death Certificate

 

To obtain a duplicate of a Czech Death Certificate please send us the following:
 

1. Completed  APPLICATION FOR DUPLICATE OF DEATH CERTIFICATE (PDF, 113 KB)(your signature on the application form must be officially  verified)
2. Copy of your passport or driving licensee
3. In case you are not a relative, enclose an explanation letter why do you need the duplicate of the marriage certificate
4. Fee

• the fee for issuance of the duplicate of the death certificate
• the fee for issuance of a verification please add extra
