Invitation

  1.  Application Form
  2. Travel document must be valid for at least 90 days after the intended departure from Schengen and will contain at least two blank pages.
  3. One (1) recent photograph, photo size should be 3.5 x 4.5 cm, with a light-colored background. The photograph must be pasted/glued on the designated space on the upper right side corner of the first page of the application form.
  4. Official Invitation Form – certified by a local branch of Czech Alien and Border Police The Official Invitation Form is available at branches of Czech Alien and Border Police.  It is up to the host / inviting person in the Czech Republic to arrange it.           
  5. Round trip flight reservation
  6. Travel medical insurance must be valid throughout the territory of the Member States and cover the entire period of the person’s intended stay or transit. The minimum coverage must be EUR 30,000. Travel insurance cannot be accepted if it excludes the coverage of a pandemic illness.
  7. Ties to the Philippines: Proof  the visa applicant has ties to the Philippines, e.g. that the applicant works or is enrolled at a university.
    1. For employees: Certificate of Employment, Leave Approval
    2. For business owners: Business papers (SEC/DTI Certification)
    3. For students: Certificate of Enrollment
    4. Even in presence of Official Invitation Form, the applicant also submits original bank account statement showing transaction history for the past 3 months, in order to prove that the visa applicant works. The amount of money in the bank account is not relevant, it is only important to demonstrate that the applicant has regular income. The Official Bank Certificate must include a paragraph containing waiver of rights to release information to the Embassy of the Czech Republic - Visa Section. If this is not possible, a separate authorization letter addressed to the bank concerned must be produced, signed by the account holder and acknowledged by the bank representative (whether by signature or by stamp), a copy of which must be submitted to the Embassy at the time of application.
  8. Minors:
    1. Affidavit of support and consent of parents (specially of the non-traveling parent) or legal guardian
    2. Copy of passport of parents or legal guardian
    3. Birth certificate of the minor certified by the Philippine Statistics Authority (PSA)
    4. Department of Social Welfare and Development (DWD) clearance
