List of requirements – short - term (Type C Visa, stay under 90days)

Applicants are required to present the original documents. If the applicant wants the document to be returned (i.e. the original birth certificate) the applicant must submit a copy of the document as well.

1. Tourism

a) Application Form

b) Travel document

Passport must be valid for at least 90 days after the intended departure from Schengen and will contain at least two blank pages.

c) One (1) recent photograph

Photo size should be 3.5 x 4.5 cm, with a light-colored background. The photograph must be pasted/glued on the designated space on the upper right side corner of the first page of the application form.

d) Accommodation Confirmation

e) Official Bank Certificate (proof of financial means)

Original bank account statement showing transaction history for the past 3 months;

The Official Bank Certificate must include a paragraph containing waiver of rights to release information to the Embassy of the Czech Republic - Visa Section. If this is not possible, a separate authorization letter addressed to the bank concerned must be produced, signed by the account holder and acknowledged by the bank representative (whether by signature or by stamp), a copy of which must be submitted to the Embassy at the time of application.

f) Round trip flight reservation

g) Travel medical insurance

Insurance must be valid throughout the territory of the Member States and cover the entire period of the person’s intended stay or transit. The minimum coverage must be EUR 30,000.

h) Other documents to support the visa application such as:

a. For employees: Certificate of Employment, Leave Approval

b. For business owners: Business papers (SEC/DTI Certification)

c. For students: Certificate of Enrollment

d. For minors: Affidavit of support and consent of parents (specially of the non-traveling parent) or legal guardian
- Copy of passport of parents or legal guardian
- Birth certificate of the minor certified by the National Statistics Office (NSO)
- Department of Social Welfare and Development (DSWD) clearance

2. Business

b) Travel document

Photo size should be 3.5 x 4.5 cm, with a light-colored background. The photograph must be pasted/glued on the designated space on the upper right side corner of the first page of the application form.

d) Official Invitation Form - certified by a local branch of Czech Alien and Border Police

The Official Invitation Form is available at branches of Czech Alien and Border Police. To view website of the Czech Alien and Border Police, click here

It is up to the host / inviting company in the Czech Republic to arrange it.        

In the absence of the Official Invitation Form:

* an invitation letter from the inviting Czech company. The Official Invitation Form is however strongly recommended.

e) Official Bank Certificate (proof of financial means)

Original bank account statement showing transaction history for the past 3 months;

The Official Bank Certificate must include a paragraph containing waiver of rights to release information to the Embassy of the Czech Republic - Visa Section. If this is not possible, a separate authorization letter addressed to the bank concerned must be produced, signed by the account holder and acknowledged by the bank representative (whether by signature or by stamp), a copy of which must be submitted to the Embassy at the time of application.

The letter must include information on the applicant’s current company designation, the nature/purpose of travel to the Czech Republic and confirmation of the applicant’s working relationship with the employer upon return to the Philippines.

b. For business owners : Business papers (SEC/DTI Certification)

6. Short-term employment

a) Application Form

d) Work Permit issued by the Czech Labor Office

Original or a certified copy of the employment permit (Rozhodnutí o povolení zaměstnat cizince) issued by a labor office in the Czech Republic. This document shall also support the application of a partner/member, the statutory body or a member of a statutory or other body of a company who performs for the company day-to-day tasks resulting from the object of business activities of the company as the legal entity, or the application of a member of a cooperative or a statutory or other body of the cooperative who performs for the cooperative day-to-day tasks resulting from the object of business activities of the cooperative as the legal entity; or

A document proving the fulfillment of the condition stipulated in Section 98 of Act No. 435/2004 Coll. on employment, i.e. that the alien's employment does not require an employment permit issued by a labor office in the Czech Republic;

* For those traveling to the Czech Republic for work / employment, it is recommended that you contact the Philippine Overseas Employment Administration (POEA) for details on required documentation, if any.
