On-line arrival form for the Czech Republic

Every traveller to the Czech Republic must fill in an on-line arrival form. It must be completed before arrival in the Czech Republic by everybody who has stayed for more than 12 hours out of the last 14 days in any country that is not on the list of countries with a low Covid-19 risk. This is not applicable to:

- employees of international transport firms,

- EU citizens,

- person with a long-term or permanent residence permit in the EU passing through the Czech Republic in 12 hours or less,

- diplomats and officials of countries or international organizations registered with the Ministry of Foreign Affairs, provided that their stay here does not exceed 14 days.

The form may be printed out or displayed on a mobile device and shown on arrival in the Czech Republic. The form also includes a section on fellow passengers, which needs only be completed if they are family members under the age of 18. These persons need not therefore complete their own form, but are part of the form to be completed by an adult.

After completion and submission of the form, a confirmation of completion will be sent to the user at the e-mail address given in the form. The e-mail contains a QR code and a link to display the entire confirmation, including all data provided. After scanning on any mobile phone, the QR code will display the on-line version of the confirmation for verification. The displayed verification contains all the data provided.

The obligation to wear face masks

The Ministry of Health is responding to the deteriorating epidemic situation is introducing the mandatory wearing of face masks, with some exceptions, in all indoor areas of buildings outside housing. The nationwide obligation to wear protective equipment in public transport also continues to apply. Face masks are basic preventive measures not only in connection with Covid-19, but also in connection with the upcoming period of respiratory diseases.

The obligation to wear face masks applies from 18 September (full text of the extraordinary measure in Czech):

1. With effect from 0:00 on 18 September 2020 until this extraordinary measure is revoked,  residence and movement without respiratory protective equipment (for the nose, mouth) such as a respirator, face mask, mouth cover, scarf, shawl or other means which prevent the spread of droplets is prohibited for everyone, specifically:
   1. inside all buildings, except for residential housing or places of accommodation (for example, a hotel room),
   2. in public transport.
2. The prohibition referred to in Point 1 does not apply to:
   1. children under two years of age,
   2. children and teaching staff in kindergartens, and children and their carers in a children's group,
   3. pupils and teaching staff of the 1st stage of an elementary school in school buildings and school facilities pursuant to Act No 561/2004, on pre-school, elementary, secondary, higher vocational and other education (the Education Act), as amended, except in their common areas,
   4. pupils, students and teaching staff pursuant to the Education Act and students and academic staff pursuant to Act No 111/1998; on Higher Education Institutions and a change and addition  to certain laws (the Act on Higher Education Institutions), as amended, within educational activities whose nature does not allow the wearing of protective equipment (especially physical training, singing, playing wind instruments),
   5. boarding schools and children’s homes, in the case of staff or the accommodated children, pupils or students,
   6. institutional care or protective care educational facilities and schools established there and educational care centres providing boarding services,
   7. schools established by the Ministry of Justice,
   8. pupils, students and teaching staff in any school established under Section 16(9) of the Education Act,
   9. employees and children in facilities for children requiring immediate assistance,
   10. persons with an intellectual disorder, an autistic spectrum disorder or a cognitive disorder or a severe alteration of their mental state, whose mental abilities or current mental state do not permit compliance with this prohibition,
   11. patients, when hospitalised in in-patient health care facilities or where this is necessary for the provision of health care,
   12. health care professionals for as long as is strictly necessary for the provision of health care,
   13. users of social services in social services facilities, such as weekly care homes, homes for the disabled, homes for the elderly and homes under special regime, and in facilities providing relief social services in residential form,
   14. other cases worthy of consideration, as determined by the health or social services provider or the doctor in attendance, for movement and residence in health care and social services facilities,
   15. employees and those in similar standing, including constitutional officials, while performing work in one place, if such a person works at a distance of at least 2 metres from another person,
   16. those driving a public transport vehicle, when they are not in direct contact with passengers during boarding,
   17. judges, assistant judges, public prosecutors, accused persons and their lawyers, participants in civil, administrative and constitutional court proceedings and their representatives, witnesses, experts, interpreters and other persons, as ruled on by the court, at the place and time of court proceedings,
   18. those performing a copyright work (e.g. a theatre, dance or music performance), those lecturing, and persons performing in the creation and production of an audiovisual work or programme,
   19. moderators, editors and other persons appearing in radio, television and other programmes,
   20. those performing work classified by the competent public health authority as third or fourth category for the risk factor of working under heat load, as well as those carrying out work which has not yet been classified and which may be expected to be classified due to the presence of a working conditions risk factor of heat load, falling into the third or fourth category,
   21. customers of catering establishments when consuming food and meals, including beverages, provided that the customer is sitting at a table,
   22. those travelling in public transport for the time strictly necessary for the consumption of food and meals, including beverages,
   23. the spouses at a wedding ceremony and other persons present at that ceremony and persons making a declaration that they are entering into a registered partnership together, and other persons present at that declaration,
   24. any persons for the time strictly necessary for the taking of a portrait photograph, or photographs of newly-weds, including any joint photograph with household members and other family and friends,
   25. sportsmen or those in training during training, exercise, contests, competition, etc.
   26. those using the premises of indoor artificial bathing arenas, such as a swimming pool, a bathing pool, a pool for infants and toddlers, or a paddling pool, spa and treatment pools and saunas.
