How to submit the application?

In person - you need to make an appointment for a personal submission by sending the request to the email address Ottawa.Consulate@mzv.gov.cz. 

By regular mail - our mailing address is

When mailing the applicatiom, your signature on the application form has to be verified by a Czech honorary consulate.

The application consists of:

1. Proof of Czech citizenship of the deceased - either (a) a certificate of Czech citizenship not older than 1 year at the time of death, (b) passport valid at the time of death (c) valid national ID card ("občanský průkaz") valid at the time of death. If there is no such proof of citizenship, please contact the embassy first.
2. Application form (DOCX, 32 KB);
3. Canadian death certificate issued by the civil vital statistics office (documents issued by funeral homes are not acceptable), apostilled and translated into the Czech language, its translation will be verified while processing your application, please do not attach the translation to the death certificate;
4. Other documents, if available - birth certificate of the deceased, marriage certficate of the deceased, valid Czech IDs, health insurance documents etc.
5. Personal data (DOC, 24 KB) form
6. a document proving the relationship to the deceased or other legal interest in registering the death in the Czech Republic, (its copy, if the application is mailed), the copy does not have to be verified and translated)
7. Applicants's valid ID (its copy, if the application is mailed), the copy does not have to be verified and translated);
8. Consular fee for the application. Please take note, that other consular fees might be connected with the processing of this application, namely for documents verifications (e.g. verification of copies or translations).

We would also appreciate receiving a pre-paid trackable envelope with your return address. We recommend using Xpresspost envelopes.
