How to submit the application?

In person - you need to make an appointment for a personal submission by sending the request to the email address Ottawa.Consulate@mzv.gov.cz. 

By regular mail - our mailing address is

When mailing your application, your signatures on the applications forms has to be verified by a Czech Honorary Consulate in Canada.

The application consists of:

1. Application form Zápis o uzavření manželství (DOCX, 55 KB)
2. Proof of the Czech citizenship of both spouses, if both are Czech citizens - either a certificate of Czech citizenship, or a valid passport or valid Czech ID card ("občanský průkaz"). If applicants do not have any of these, they must first obtain the Czech Citizenship Certificate;
3. The Canadian/foreign marriage certificate (wallet form is not accepted), apostilled and translated into Czech language;
4. Birth certificate of a spouse if he is a Czech citizen;
5. In case a Czech party to the marriage enters it as a widow/-er it is necessary to submit a death certificate of his/her previous spouse. In case a Czech party to the marriage enters it as a divorcee, a divorce certificate from the previous marriage must be submitted (it must contain the date the divorce took effect), apostilled and translated into Czech language;
6. If applicable, the application form (PDF, 310 KB) to register the female name in its neutral form (without the suffix "-ová" at the end); There is the possibility to apply for the registration of the female surname in its neutral form additionally as well, using this form; (PDF, 311 KB)
7. Personal data (DOC, 24 KB) form;
8. Applicants's valid ID (its copy, if the application is mailed, the copy does not have to be verified or translated);
9. Consular fee for the application. Please take note, that other consular fees might be connected with the processing of this application, namely for documents verifications (e.g. verification of copies or translations). 

We would also appreciate receiving a pre-paid trackable envelope with your return address. We recommend using Xpresspost envelopes.
