Application for Certificate of Czech Citizenship (CCC)

PURPOSE OF THE DOCUMENT:

A citizenship certificate issued in the last 12 months is a proof of Czech citizenship. Its possession is not mandatory as a valid passport or national identity card ("občanský průkaz") can be used as a proof of Czech citizenship. However, it is necessary to obtain and submit a citizenship certificate when a person wishes to apply for a Czech passport abroad and does not have any other proof of citizenship issued in the Czech Republic (valid passport, občanský průkaz, or pre-marriage approval containing the citizenship status).

PLEASE NOTE: Pursuant to the Czech Citizenship Act, birth certificates, marriage certificates and drivers licences are not valid as a proof of citizenship. Also, a CCC older than 12 months cannot be accepted as valid proof of citizenship.

A. APPLICANTS BORN IN THE CZECH REPUBLIC, CZECHOSLOVAKIA AND ITS PREDECESSORS NEED TO SUBMIT:

1. Application for Certificate of Czech Citizenship, questionnaire for Certificate of Czech Citizenship for Minors, questionnaire for Certificate of Czech Citizenship for Minors (15 - 18 y.o.), questionnaire for Certificate of Czech Citizenship for adults

PLEASE NOTE: The application form must be filled out completely. If a particular question is not applicable, please indicate this in the form. Do not leave blank spaces.

2. Czech or Czechoslovak birth certificate

PLEASE NOTE: If the applicant has changed their marital status or their name due to marriage and this change has not yet been registered by competent authorities in the Czech Republic, an authenticated and translated marriage certificate is also required. In the event that don 't have a Czech or Czechoslovak birth certificate (due to loss, theft or destruction), you can apply for its issuance. The necessary application form - duplicate

3. Dated Certificate of Citizenship of all the countries that an applicant is a citizen of (e.g. certificate of naturalisation in SA).

B. APPLICANTS BORN ELSEWHERE THAT HAVE BEEN PREVIOUSLY ISSUED PROOF OF CZECH CITIZENSHIP:

If the applicant is born abroad but has a birth certificate issued in the Czech Republic, all of the above named documents are requested

Applicants that only possess foreign birth certificates must have these authenticated and translated into Czech before submitting them to the Embassy. More information on obtaining Czech birth certificate

C. APPLICANTS BORN ELSEWHERE THAT HAVE NOT HAD PREVIOUSLY ISSUED PROOF OF CZECH CITIZENSHIP NEED TO SUBMIT:

Persons who were born abroad to a Czech citizen (referred to as "parent" or "parents" further in text), who have never had proof of Czech citizenship need to submit the following documents:

1. Application for Certificate of Czech Citizenship, questionnaire for Certificate of Czech Citizenship for Minors, questionnaire for Certificate of Czech Citizenship for Minors (15 - 18 y.o.), questionnaire for Certificate of Czech Citizenship for adults

PLEASE NOTE: The application form must be filled out completely. If a particular question is not applicable, please indicate this in the form. Do not leave blank spaces.

2. Czech Birth certificates of applicant 's parent - if both parents are Czech citizens, birth certificates of both are required as well as Czech parent citizenship affidavid.

3. Czech marriage certificate of applicant ' s parents (if they were married at the time of applicant 's birth)

PLEASE NOTE: If you only have a marriage certificate issued in SA or any other foreign country, it must be authenticated and translated into Czech. More on obtaining the Czech marriage certificate

4. Foreign birth certificate of the applicant - this document must be authenticated and translated into Czech before submission to the Consulate. More information on obtaining Czech birth certificate

PLEASE NOTE: When obtaining the CCC for a minor child, the child still remains the applicant. Application forms must be signed by a parent or legal guardian on behalf of the minor child.

PROCESSING TIME:

In most cases the Certificate of Czech Citizenship is issued within 1 to 2 months if the applicant was born in the Czech Republic and has or previously had a proof of citizenship (ie. passport, ID card...). If this is not the case, expect about 3 to 6 months waiting period.

Application fee is paid in cash.

ADDITIONAL INFORMATION:

In addition to the mentioned documents, all applicants are also requested to submit a Personal Data Form and must prove their identity with valid ID or passport.

Application can be made for this document at our office in person.

Please understand that the consular staff cannot fill out the application forms for you. However, we will gladly go assist you with the forms and answer any questions you may have.
