If you do not possess the vital certificate, it is possible to apply for a duplicate. Duplicates can be issued only if the birth, death or marriage was registered in the Czech Republic .

The Embassy of the Czech Republic in Pretoria requires all applicants to register prior to visiting the office. Appointment can be made via email: pretoria.consulate@mzv.gov.cz

Duplicates can only be issued to an eligible applicant: the applicant themselves, a relative (proof must be shown), or for legal purposes (must be demonstrated).

You must submit:
1. A completed application form.
2. A valid ID or passport
3. If you are not the person named on the certificate, you must explain why such document is needed including copies of any suporting documents.
4. A completed form: Osobní údaje

Turnaround time is on average 5-10 weeks

Consular fee must be paid in cash.

Once the Consulate receives the document you will be contacted.
