Obligation of passengers traveling to the Czech Republic to fill in the online form before entering the country

Passengers that are not on the list of countries with a low risk of COVID-19 infection are obliged to fill in an online form for the hygiene station before entering the Czech Republic. The form is avilable on following website: www.prijezdovyformular.cz

Due to the adverse epidemiological situation in terms of the incidence of the COVID-19 and related activities of public health authorities conducted in the interest of protecting public health, the provision of the data specified below is required. By providing them, you will make it possible for public health authorities to contact you, should it be discovered that you may have been infected by another person during your trip or that you yourself may be a source of infection for other passengers. Your personal data will be processed exclusively for the purposes of exercising public authority in the sphere of public health protection. The processing of your personal data is governed by Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC and by Act No. 110/2019 Coll., on personal data protection, which was enacted in compliance with Regulation (EU) 2016/679 of the European Parliament and of the Council.

The joint controllers of personal data are the Ministry of Health and the regional public health station. The legal basis for the processing of personal data is Section 79 of Act No. 258/2000 Coll., on public health protection and amending certain related acts and protective measures of the Ministry of Health, issued pursuant to Section 80(1)(h) of Act No. 258/2000 Coll. for the purpose specified above.

You can request from the Ministry of Health access to your personal data, i.e., information about specifically what personal data of yours we process, and demand their rectification, deletion, restriction of processing, or object against processing. Should you think that the processing of your personal data violates the law, you can lodge a complaint with the national supervisory authority – the Office for Personal Data Protection (www.uoou.cz). The obligation to provide personal data follows from the applicable protective measure of the Ministry of Health. Your personal data will be processed for a period of 2 months
