Requirement for presenting Extract from the Penal Register record

The Consulate General of the Czech Republic in Shanghai requests Extract from the Penal Register record to be presented by applicants for long-term visa or resident permit to present.

The Consulate General of the Czech Republic in Shanghai, with accord with § 31,article 3 paragraph a) of the Act No. 326/1999 Coll., On the residence of foreigners in the Czech Republic, request all long-term visa or long-term residence permit applicants, who are about to lodge their application at the Consulate to present Extract from the Penal Register or similar, issued by issued by the state of which the foreign national is a citizen and also by the states in which the foreign national has had continuous residence for over 6 months in the last 3 years. The extract from the Penal Register record may be replaced by an affidavit, if the given state does not issue such a document. The obligation to submit an extract from the Penal Register record does not relate to a foreign national under 15 years old.
