Necessary elements for submitting an application (attention to the validity of documents!):

1. Proof of identity:

• - Valid Czech passport,
• - valid Czech identity card,
• - valid ID card of a Member of the Parliament of the Czech Republic;

2. Proof of the name and surname to be entered in the passport:

• - valid Czech passport,
• - valid Czech identity card,
• - Czech civil registry document (e.g. birth/marriage certificate);

3. Proof of Czech citizenship:

• - valid Czech passport,
• - valid Czech national identity card,
• - if the applicant does not have a valid Czech passport or an ID card, a valid certificate of Czech citizenship must be provided, i.e. a certificate issued within 1 year before the passport application submission - for information on the procedure for processing the document, see another section of our website);

4. Proof of birth number:

• - valid Czech passport,
• - valid Czech ID card,
• - Czech birth/marriage certificate,
• - separate proof of birth number (see other section of our website for information)
• - If the applicant does not have a Czech birth number, he/she must first apply for one or have it verified (information in another section of our website);

5. 1 passport photo in 5 x 5 cm format

• - more detailed requirements here: Biometric passport photo guide (PDF, 433 KB) (please disregard the sizes mentioned in the document, other criteria do apply!)
• - Important notice: the Czech electronic passport system has high requirements for the quality of the photo, we highly recommend a high quality photo from a photo professional, not from a "photo booth" which the system may not accept! We also recommend obtaining a digital backup of the photo.

7. Alternatively: proof of change or document confirming new data to be inscribed

• - if the reason for issuing a new passport is a change in one of the registered data (e.g. marriage certificate in case of change of surname after marriage)

Important note: If you wish to collect your passport elsewhere than at our Embassy (e.g. at a municipal office in the Czech Republic or at another Czech embassy), you must inform us of this when submitting your application!!!

 

Important information as of 15 September 2022: Due to more lenient methodology of the Ministry of the Interior of the Czech Republic, a valid (i.e. maximum 1 year old) certificate of citizenship is NO LONGER REQUIRED in case a citizen holds an invalid Czech passport with biometric elements or a Czech ID card if either of these documents was issued after 1 January 2014. It is also possible to accept one of these expired documents issued before 31.12.2013 IN COMBINATION WITH a certificate of citizenship of the Czech Republic issued after 1.1.2014 (i.e. already expired). The same procedure applies to minors born after 1.1.2014 who present a certificate of citizenship of the Czech Republic issued after 1.1.2014 (i.e. already expired). Please note that a citizenship certificate will still be required in cases where it is unclear whether the citizen has lost Czech citizenship in the meantime.

 

Application for minors:

Requirements:

1. See above points 1 to 7
2. In addition: the child's birth certificate and a valid identity document of the parent (passport or ID card of one of the parents).
3. Minors over 15 years of age must attach to the application a written consent of the parent or legal guardian for the issue of a passport. This consent can be signed by the parent/legal guardian in front of the consular officer at the time of application. If the application for a passport is made by a minor over 15 years of age in the absence of a parent/legal guardian, their signature on the consent must be notarised.

Please note: If the minor does not have a valid Czech passport or ID card, it will be necessary to present a valid certificate of Czech citizenship , i.e. a certificate issued within 1 year before the passport application submission - for information on the procedure for processing the document, see another section of our website).

Application to be submitted by:

• - Only a parent/legal guardian (e.g. foster parent, etc.) can submit the application for children under 15 years of age.

Handing over the completed passport:

• - To the parent/legal guardian (i.e. without the personal presence of the child who has applied for the passport),
• - or to the applicant over 15 years of age.
