Only concerned person and direct relatives of concerned person (husband, wife, father, mother, child, sibling, grandchild, and grandparent or person with another legal reason) are authorised to apply for a duplicate Czech Birth, Marriage or Death Certificate. If unrelated third person (or institution) requires duplicate of the document a document of relationship or  a legal prove must be included.  The i authority who decides if the applicant is entitled is the competent Registry in the Czech Republic.

Please note, that for each document (birth/ death/ marriage  certificate) a  separate application form must be filled out!

To obtain a duplicate of Certificate please send us the following:

1. Completed application form ( if  not submitted in person  at the  Consulate, the signature on the application form must be verified by a Justice of Peace)

2. Signed "Consent to the Processing of Personal Data" form with contacts

3. Copy of valid passport  of the applicant or driving licence (verified by a Justice of Peace)  

4.  the fee (code 153a for issuance of the duplicate  and code 150e if apostille is also required). Instruction for payment will be sent when receiving the  complete application.

In order to assist those who do not speak Czech, we have included a Guide Form, however please note that this form serves as a guide only! DO NOT FILL OUT OR SUBMIT THE ENGLISH VERSION OF THE FORM. Applicants MUST submit the completed Czech version of the form as per above instructions.
