Who can apply for the Czech version of the EU COVID Digital Certificate:

1. persons covered by the Czech public health insurance
2. persons entitled to the same health care as if they were covered by the Czech public health insurance
3. persons who are not covered by the Czech public health insurance, but are:
   1. citizens of the Czech Republic
   2. foreigners who have resided in the Czech Republic for more than 90 days and at the same time meet one of the following conditions:
      • ● have a valid long-term or permanent residence permit in the Czech Republic
      • ● are in possession of a valid long-term visa
      • ● are EU citizens with a certificate of temporary residence or a permanent residence permit in the Czech Republic
      • ● are a family member of an EU citizen with a certificate of temporary residence or a permanent residence permit in the Czech Republic.

If you are interested in issuing an EU COVID certificate, please contact your general practitioner in the Czech Republic. If your GP is registered in the national vaccination system, he/she can - upon presenting a Japanese certificate - enter your personal details in the national vaccination register. If the practitioner does not have access to the Covid-19 vaccination system, you can contact the nearest vaccination centre, which will administer the registration.

Kind reminder: As of December 20th, 2021 Japanese Covid-19 certificates have been recognized by the Ministry of Health of the Czech Republic. Their holders are therefore considered to be fully vaccinated and can travel to the Czech Republic for any purpose (without any need to obtain the EU COVID Digital Certificate).
