Application for Czech Citizenship Certificate (CCC)

Purpose of this document:
Citizenship certificate issued in last 12 months is a proof of Czech citizenship. Its possession is not mandatory as a valid passport or national identity card ("občanský průkaz") can be used as a proof of Czech citizenship. However, it is necessary to obtain and submit citizenship certificate when a person wishes to apply for a Czech passport abroad and does not possess other proof of citizenship issued in the Czech Republic (valid passport, občanský průkaz).
PLEASE NOTE: Pursuant to the Czech Citizenship Act, birth certificates, marriage certificates and drivers licences are not proofs of citizenship. Also, a CCC older than 12 months cannot be accepted as valid proof of citizenship.
 

For people born abroad

If you wish to find out about applying for Czech citizenship, please read carefully. The citizenship legislation has undergone many developments/changes. Your application will be considered according to the citizenship legislation that was in place at the time of your birth.

The Consulate may advise; however, it is not authorized to declare whether a person is or is not a Czech Republic citizen. Such a decision is made by the appropriate authority in the Czech Republic and only after submitting and application and supporting documents – see below. Without all the necessary supporting documents submitted in the required format the application cannot be processed.

If a person was born outside the CR to a Czech parent or parents, a Czech citizenship certificate application must be submitted first to find out/confirm the Czech citizenship.

It is not possible to apply for a passport right away. The citizenship must be established first.

This means that you will be submitting your application for a Czech citizenship certificate first, followed by the application to register your birth and possibly marriage/divorce.

Only after the birth, marriage/divorce etc. have been registered, an application for passport may be submitted.

 

Documents considered by law as proof of Czech citizenship:

a) valid Czech national ID (obcansky prukaz)

b) valid Czech passport

c) Certificate of Czech Citizenship (Osvedceni o statnim obcanstvi) not older than one year from the date of issue  

d) Document on granting Czech Republic Citizenship (Listina o nabyti statniho obcanstvi Ceske republiky) – not older than one year since the granting of citizenship

 

General steps to follow:

Under the age of 15 - applications are submitted by a parent or a legal guardian  

Over the age of 15 - applications are submitted by applicants themselves

Leaving items blank may delay the processing and may result in the application being returned. If there is an item that is not applicable to your situation, it must be indicated on the form.

Dates are to be written in the format Day/Month/Year

  • A Consular officer will review the form and may be able to answer your questions but is unable to fill the forms out for you.


 

The following must be submitted

Please note: All documents must be originals. Copies of documents, excluding those certified in the Czech Republic, cannot be accepted.

  1. Completed form: Žádost a dotazník ke zjištění státního občanství a vydání osvědčení o státním občanství České republiky.
  2. Applicant’s birth certificate. To obtain the certificate, you must contact the appropriate provincial authority:

Ontario: Service Ontario

Alberta: Vital Statistics Alberta

Manitoba: Manitoba Vital Statistics Agency

Saskatchewan: eHealth Saskatchewan

 

  • The birth certificate MUST contain parents’ names and surnames. Please obtain the long-form format birth certificate (also referred to as certified copy of birth registration) as it contains all the necessary information. We are unable to accept wallet size birth certificates or certificates that do not contain information on parents.
  • The original (NOT a notarized copy) must be affixed with a certification called Apostille by the appropriate government authority – this depends on where the birth certificate was issued:

  • Alberta: Deputy Provincial Secretary’s Office
  • Manitoba: Global Affairs Canada
  • Saskatchewan: Ministry of Justice and Attorney General of Saskatchewan

 

  • Along with the original birth certificate affixed with an Apostille you will submit a professional translation into Czech language. The translation must be done on a computer on a separate sheet of paper. The translation does not have to be notarized. We do not accept translation by automated translators.

 

As a part of the citizenship application process, once it has been determined that you are a Czech citizen, you will also be applying for a birth registration, see link here. The same official documents required for citizenship application are also required for an application for birth registration.

  1. Applicant’s marriage certificate if married; if divorced, divorce certificate that contains the exact date when the ruling took effect; or death certificate of the spouse if widowed. In the case of a marriage, divorce or a death that took place in Canada, the documents must be issued by the provincial government authority, affixed with an Apostille certification and accompanied by a professional translation into Czech language.  If you were married or divorced in another country, please contact us for details. As a part of the citizenship application process, once it has been determined that you are a Czech citizen, you will also be submitting an application for a marriage registration, see link here
  2. Czech birth certificate of the applicant’s parents
  3. Czech marriage, and divorce certificate of the applicant’s parents if applicable
  4. Czech death certificate of the applicant’s parents if available
  5. Any Czech/Czechoslovak citizenships documents issued to the applicant’s parents in the past if available
  6. Citizenship certificates of all countries your Czech parents were citizen of – for example, Canadian citizenship certificate - see point 10.
  7. Adoption certificate if adopted by a Czech citizen.
  8. Citizenship document(s) of all countries you are a citizen of unless you were born in Canada and therefore have your birth certificate as a proof of your Canadian citizenship. These documents must be originals and depending on the country of issue, legalized and translated into Czech language. If unsure, please contact us beforehand. In the case of Canadian citizenship, we require a paper Canadian citizenship certificate (Commemoration certificate) that includes the exact date of becoming a citizen as well as the citizenship card. ONLY submitting a citizenship card is not sufficient as it contains only the date of issue.
  9. Valid picture ID

For people born in the Czech Republic, Czechoslovakia or those whose birth abroad has already been registered. i.e., applicants who have been previously issued a Czech birth certificate.

Your birth has already been registered and you have a Czech birth certificate. If you do not hold a valid Czech passport or valid “obcansky prukaz”, you will need to apply for a Czech citizenship certificate to confirm your Czech citizenship and to update your marital status or change your name/last name if this occurred (such as registering your marriage or divorce abroad if you have not already done so in the past). When all your documents are up to date, you will be able to apply for a Czech passport.

 

SITUATIONS WHEN IT IS NOT REQUIRED TO OBTAIN CZECH CITIZENHIP CERTIFICATE FOR THE PURPOSE OF PASSPORT APPLICATION:

 

  1. You are a holder of a passport or občanský průkaz issued after on or after January 1, 2014

 

  1. In case of a Czech citizen who was born abroad after January 1, 2014 who submits an original Czech citizenship certificate issued after January 1, 2014 (even though no longer valid) or a Czech birth certificate issued by zvláštní matrika Brno after January 1, 2014.

 

  1. In case of a Czech citizen who is a holder of expired Czech passport (e-pas) or občanský průkaz issued by December 31, 2013 and submits  Czech citizenship certificate  issued after January 1, 2014(even though the certificate is no longer valid)

 

Documents considered by law as proofs of Czech citizenship

a) Valid Czech national ID (obcansky prukaz)

b) Valid Czech passport

c) Certificate of Czech Citizenship (Osvedceni o statnim obcanstvi) not older than one year from the date of issue  

d) Document on granting Czech Republic Citizenship (Listina o nabyti statniho obcanstvi Ceske republiky) – not older than one year since the granting of citizenship

 

General steps to follow:

  • If applications are mailed, the applicant’s signatures on the forms MUST be verified by the Honorary Consulate in your area. PLEASE NOTE, if you are submitting a photocopy of a Czech passport or obcansky prukaz, the holder must date and sign the copy and include a written acknowledgment that the ID has been photocopied for the purpose of the application: "Souhlasím s pořízením kopie mého dokladu“
  • Application forms must be filled out correctly, legibly, in Czech language and nothing is to be omitted.

  2. Czech birth certificate
  3. Czech marriage certificate if married; if divorced, Czech divorce certificate that contains exact date when the ruling took effect; or death certificate of the spouse if widowed. In the case of a marriage, divorce or a death that took place in Canada, and has not been registered in the Czech Republic, the original Canadian certificates must be affixed with an Apostille and accompanied by professional translation into Czech language.   4. Czech birth certificate of the applicant’s parents if available
  5. Czech marriage, or divorce certificate of the applicant’s parents if available
  6. Czech death certificate of the applicant’s parents if available
  7. Citizenship document(s) of all countries you are a citizen of unless you were born in Canada and therefore have your birth certificate as a proof of your Canadian citizenship.
