Prevalidation of your name in the central database for a passport renewal

If you changed your name we recommend that you contact us before coming to apply for a new passport.

In order to apply for a biometric passport, it is necessary that the applicant's personal data is up-to-date in the central database system called "Czech Citizenship Register Database (ISEO)". From time to time a situation may occur (usually after changing name)  when an applicant does not have these data updated or he/she is not in the database at all. In this case, it is necessary first to correct this situation and only then carry out the passport application procedure.

To avoid possible problems, we recommend the applicants (especially those traveling from afar) to send this application form together with a copy of your Czech ID to the Consulate by  email. We can check your data and confirm that they are correct and up-to-date.
