To obtain such a document fulfill the following requirements:

1)The original/downloaded application form, please, fill out the following columns:

• Name and Surname at Birth
• Present Surname
• Date of Birth
• Place of Birth
• Birth Number - for those born in the Czech Republic
• Name and Surname of Father
• Name and Surname of Mother
• Mother's Given name
• Your signature

The application can be launched in person or by mail (in that case we require the signature to be verified by a Notary Public). Please, be ready to prove your identity.

The processing time is approximately two months.

The fee is collected only in cash in EUR according to the exchange rate of the Czech National Bank valid for the respective months (see the fixed fee in Czech Crowns in the table).
