Due to organisational reasons the applicants are kindly requested to arrange the appointment at the Consular Section of the Embassy in advance by e-mail at the address: visa_warsaw@mzv.cz .

Foreigners interested in submitting a visa/residence permit application may request an allocation of a term of appointment on the 10th day of each month starting at 00.00:01 a.m.  for the following month. In case that the 10th day of a month is not a working day in the Czech Republic, you may ask for an allocation of a term of appointment the first following working day starting at 00.00:01 a.m.

In the request the following data must be stated: name, surname, birth date, citizenship, the information whether a foreigner is a holder of the Polish, Lithuanian, Latvian, Estonian, Norwegian, Swedish, Finnish or Icelandic residence permit card, what is the purpose of the visa/residence permit application. Furthermore the interested person is obliged to include to his/her e-mail a scan of a document proving the purpose of stay – a document, which will be submitted at the Embassy together with the application. Moreover the interested person is obliged to include to his/her e-mail a scan of the residence permit card, if the obligation to submit such a document is relevant in his/her case.

In one e-mail just one applicant can be mentioned; this rule doesn’t apply to members of one family.  We would like to draw your attention to the fact that the number of foreigners interested in submitting their applications substantially exceeds the Embassy`s capacity.

In principle, the Embassy is able monthly to allocate slots to 73 interested persons, who intend to submit an application for a long-term visa or a residence permit. This figure may every month slightly differ depending on a number of working days or, eventually,  due to  organisational reasons on the Embassy's side.

The communication with applicants can be conducted at the Consular Section of the Embassy not only in the Czech language, but in the Polish or English languages as well. The applicant, who doesn't communicate in the mentioned languages, is obliged to arrange an interpretor on his/her in own.

The administrative fee for submitting a visa – or residence permit application is to be settled by the applicants by cash in EUR.

Fees collected for submission of long-term visa / residence permit application:

Long-term visa (including study)                   – 2.500,- CZK

Long-term visa – business purpose               – 5.000,- CZK

Residence permit                                               – 2.500,- CZK

Employment card / Blue card                         – 5.000,- CZK
