Information on long term Visa procedure
Long term (National) Visa (D type)
Long-term visa applications are submitted to the diplomatic missions of the Czech Republic according to the consular territorial competence. The diplomatic mission forwards the application to the Ministry of the Interior for an evaluation and decision. If the application is evaluated positively, the long-stay visa, with the maximum duration one year, is issued by the diplomatic mission where the application was filed.
Long-term visa application is filed when the expected stay will exceed 90 days (cannot be covered by a Schengen (short-stay) visa). It is issued for a variety of purposes – health, culture, sport, visit (invitation), family, study, research or entrepreneurship.
If the purpose of stay continues, it is possible to apply on the territory of the Czech Republic at the Ministry of the Interior for an extension, i.e. for a long-term residence permit, which is issued for the purpose of employment, study, scientific research, unification of family or to residents of another EU Member State.

- proof of accommodation;
- document on the purpose of stay;
- proof of funds;
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:
- medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll).
List of supporting documents for long term visas applications with the purpose:
STUDY
- filled long-term visa form;
- travel document (original);
- 2 photographs (with name, surname, date of birth and signed);
- proof of accommodation;
- document on enrolment in studies;
- proof of funds;
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:
- medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll.).
BUSINESS VISITS
- filled long-term visa form;
- travel document (original);
- document on the purpose of your stay (a document confirming the entry into the pertinent business register, list or record);
- proof of funds;
- proof of accommodation;
- 2 photographs with name, surname, date of birth and signed);
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:

medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll.).
FAMILY REUNIFICATION
- filled long-term visa form;
- travel document (original);
- document on the purpose of your stay (e.g. a birth certificate, marriage certificate, a document on guardianship, a document confirming the fact that the person is without any financial means, a document on a foreign national being solitary or his/her health status);
- proof of accommodation;
- proof of finance;
- 2 photographs (must be with name, surname, date of birth and signed);
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:
- medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll.).
INVITATION
- filled long-term visa form;
- travel document (original);
- invitation to visit the Czech Republic (verified by the Foreign Police Inspectorate) – document proving the commitment of the inviting person, that he/she will cover expenses for:
-
subsistence for the invited person for the whole duration of his/her stay on the territory until departure;
-
accomodation of the invited person for the whole duration of his/her stay on the territory until departure;
-
granting a medical care of the invited person for the whole duration of his/her stay on the territory until departure, including a transport of the ill person to the hospital or his remains;
-
other expenses necessary for the stay of the invited person and his departure from the territory (the cost incured to CR arising from a stay and a departure within the administrative expulsion of the foreigner);
- proof of accommodation;
- proof of finance;
- 2 photographs (with name, surname, date of birth and signed);
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:
- medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll.).
CULTURE
- filled long-term visa form;
- travel document (original);
- document on the purpose of your stay – the confirmation of the natural or legal person pursuing cultural activities on the Czech territory;
- proof of accommodation;
- proof of finance;
- 2 photographs (must be with name, surname, date of birth and signed);
- document similar to an extract from the Penal Register record, issued by the state of which you are a citizen, as well as the states in which, in the last 3 years, you have resided continuously for a period longer than 6 months, or a signed affidavit in the event that this state does not issue such a document (not required from a foreign national under 15 years).
Upon request you are further obliged to submit:
- medical report, that you do not have a serious illness; (i.e. the illnesses presented in Decree No. 274/2004 Coll.).
A visa for the purpose of collecting a residence permit – D/VR
In some cases, for example, when the passport was changed in the country of origin after or if the validity of a long term visa/residence permit expires in the course of proceedings on a legitimate newly filled application, or after approving applications for a long term residence permit filled at the Embassy, then to return/enter the Czech Republic, you will need a visa for the purpose of receiving a residence permit.
In this case you must fill in the Long-term visa form and add your photograph, no other requirements are necessary.
Important notice for all purposes
If your application is approved, prior to affixing the visa, you are obliged to submit a proof of medical travel insurance that corresponds to the specified conditions (not required if the foreign national subscribes to public health insurance or if the costs for health care are covered on the basis of an international agreement or if the foreign national can demonstrate that health care is covered in another manner), and, upon request, a document on paying the insurance presented in the proof of medical travel insurance.
All presented documents must be translated to the Czech language and must be certified by a notary public.
